Powder through expander die sometimes leaves a cutout dimple on case mouth

The damage to the cases is caused by the case mouth hitting the expander portion of the expander sleeve. The expander has a rounded leading edge that will find its way into the case so that the flare can be applied to the case mouth for bullet seating. If you make sure that the case is completely in the shell holder, and slow down your speed on the handle as you raise the case into the powder through expander die, the problem will correct itself.
