Two things to do to fix this:
1. Pull out the index pin, wipe it off, then give it a coat of oil and replace it.
2. Case ejector - the piece of plated steel on top of the shell plate. There is a tail that is bent so that it drops down into the hole in the shell plate as it indexes. Make sure the end of the tail is bent down so it does drop into the hole. You can also bend the whole tail closer to the square hole in the center of the ejector. This can be done with a needle nose pliers. One jaw in the hole and the other bending the tail closer to the hole. If you bend it in to much it will not drop into the hole. Then just bend it back out until it does drop in.
